The World Health Organization (WHO) has recently released its latest 
"H5N1 avian influenza: Timeline of major events"

Date     Events in Animals (incl. birds)                 Events in Humans

The only event in the Western Hemisphere appears to be 
6 September 2006 -  The USA detects LPAI H5N1 in wild
ducks in Pennsylvania and Maryland.

The most recent human cases have been in 
Indonesia 
Egypt
Vietnam

Percentage of cases resulting in death remains above 50%.

It appears that none have recently been transmitted human to human, only from 
bird to human.
